Creatinine in a Powerlifter/Runner’s bloodwork

Hello Jordan. A snappy query for you. I’m every other of what appears to be a proliferation of in any other case wholesome trainees of the sports of powerlifting, bodybuilding and other strength pursuits presenting raised creatinine levels in standard bloodwork.


My own numbers were 90umol/l 3 years ago, 102umol/l 9 months ago and 122umol/l last month. Excuse the european units, in american 122 umol is about 1.38 mg I believe.


Along with my most recent bloods which presented the highest creatinine level an ACR urine test was performed. This came back as a 1.1. This was described as “reassuring”. This would appear to contradict the Creatinine serum test and indicate healthy kidneys… I think. I believe the ACR is done in place of the awkward 24h urine collection test, is it comparible from an accuracy perspective?


I am a 220lb powerlifter who also does quite a bit of running, I’ve been training for about 8 years and brought my squat up over the 600lb last year so I have some meat on my bones. Interestingly in the months preceeding my highest test result i was supplementing with creatine. I stopped approx 3 weeks prior to the test. I didnt train 24 hours before the blood test, though I did quite a bit of running and some soccer in the previous days. I hadn’t done much lifting in the week or two beforehand as I was without a place to train. Running and soccer do leave me pretty stiff an sore but I’m not sure if they wold break down muscle and elevate creatinine like lifting would, it certainly feels like they do!


So there were several questions that my GP didnt really know how to answer. He’s a young guy but his clientele would be quite elderly, it’s quite a mature neighbourhood. My training and muscle mass is clearly on a different level to what he deals with.


Is the raised creatinine down to my muscle mass? Did my training elevate it in the week running up to the test? Did the months of creatine supplementation contribute to the increased serum creatinine? How long does it take for creatinine in the blood to return to baseline after supplementation with creatine if it does have an effect? Does the good ACR result negate the creatinine test and prove good kidney function?


The answer to all of these questions appeared to be “dont know”


Of course kidneys are not to be trifled with. Any damage, however small, is permanent I believe. The reason I ask for advice from you is that literally 24 hours after my GP visit to receive my results I left Ireland to work in the Caribbean. So I don’t really have much chance to follow up and I believe healthcare where I am is pretty hit and miss. I really just want to know if this needs follow up pronto or if it can wait for 6 or 9 months for my next scheduled bloods (my father handed at forty five from a coronary heart assault, I’m 34 so I control issues and have accomplished when you consider that turning 30)
