Yes... That's realistic expectations for somebody your age (Doesn't mean it will always work but for most teen it should work) Now you will need to eat enough calories to in sustained caloric surplus which is needed to gain weight... Ideally you want gain 0.5 to 1lbs a week. Which means eating 300 to 500 calories over maintenance everyday...
As a workout any 3 times a week full body routine that focus on compound lift like Starting Strength or Strong Lifts should work... Just eat a lot!
